Although her initial evaluation showed elevated inflammatory markers, further examination revealed distinctive findings consistent with Palmar Fasciitis and Polyarthritis Syndrome (PFPAS), a rare condition that can be associated with underlying cancer. Recognizing several concerning clinical features, Dr. Chawla pursued an expedited evaluation that ultimately led to the diagnosis of stage IIB high-grade serous ovarian carcinoma. He reviews the patient's presentation, diagnostic workup, treatment course and the important relationship between PFPAS and ovarian cancer. This case highlights how unusual rheumatologic symptoms can sometimes serve as the first clue to an underlying malignancy and underscores the importance of maintaining a broad differential diagnosis.
