Real assessment means sorting through lookalikes, understanding history, and looking beyond a short attention checklist. This video explores how clinicians should approach Adult ADHD diagnosis by digging into behavior patterns, development, context, daily functioning, and the many conditions that mimic or overlap with attention problems. A big part of the challenge is teasing apart ADHD misdiagnosis from the real thing. Executive dysfunction can show up in anxiety disorders, depression, bipolar disorder, PTSD, or personality disorders, but it looks and feels very different when it has been lifelong ADHD. That’s why a comprehensive ADHD evaluation considers school history, childhood behavior, adult coping styles, and environmental demands. We look at inattentive ADHD that was never caught in childhood, subtle masking strategies, and how high-achieving adults can hide symptoms until their life finally becomes too complex for their compensatory skills to keep up. We also examine ADHD vs anxiety and ADHD vs depression, which can mimic distractibility through racing thoughts or low motivation. ADHD vs bipolar disorder is especially tricky because both can include restlessness and impulsive decisions, but their time course and triggers are distinct. ADHD vs borderline personality disorder is another complex differential diagnosis; both can show emotional dysregulation, rejection sensitivity, and chronic overwhelm, yet the underlying patterns and causes differ. ADHD vs trauma also requires careful attention, since hypervigilance and dissociation can look like inattention from the outside. Good practice means slowing down, listening closely, and tracking symptoms over time instead of labeling based on a snapshot. Executive dysfunction isn’t just about being forgetful. It touches planning, prioritizing, time perception, sustained effort, and working memory. And while stimulant medication can be life changing for those with real ADHD, treatment isn’t just about pills. Holistic ADHD treatment includes building exeWhat people say.
